At Trinity Hospice, we believe every person deserves compassionate, dignified and individualised care at the end of life. We are looking for a caring and dedicated Health Care Assistant to join our Community Nursing Team, supporting patients and their families in their own homes.
As part of a multidisciplinary health and social care team, you will provide practical, personal and emotional support to patients in the last year of life. Working alongside specialist nurses and other healthcare professionals, you will play a vital role in helping patients remain comfortable, safe and well supported in their preferred place of care.
What you’ll do
- Deliver high-quality personal and practical care to patients in their own homes.
- Support admissions, discharges and ongoing care packages.
- Assist patients with washing, dressing, mobility and continence care.
- Monitor patients’ wellbeing and report any concerns or changes promptly.
- Provide comfort, reassurance and support to patients, families and carers.
- Contribute to care planning, handovers and multidisciplinary team meetings.
- Support patients and families through difficult conversations and bereavement.
- Maintain accurate clinical records and uphold the highest standards of confidentiality and dignity.
