Job Overview

Fixed term post until 28.02.2027 to cover a secondment. Only staff currently employed by Betsi Cadwaladr University Health Board are eligible. Welsh students must apply via Streamlining.

About the Role

You will be a Band 5 Community Nurse in our District Nursing Service, supporting patients in Anglesey and Gwynedd. Key responsibilities include delivering clinically effective care, communicating with the District Nurse Team, traveling between patients' homes, and contributing to high quality evidence based care.

Main Duties
  • Provide patient‑centred care within the District Nurse Team.
  • Travel between patient homes in towns and rural areas promptly.
  • Support the delivery of evidence based care to maintain patients in their own homes.
Career Development Pathways
  • Progress to Band 6 District Nurse by undertaking the District Nurse Qualification.
  • Develop as a Chronic Condition Case Manager through disease‑specific training.
  • Advance to Nurse Practitioner via modular clinical skills training.
Qualifications
  • Registered nurse or within 2 months of gaining PIN.
  • Knowledge of Community Care.
  • Practice supervisor/assessor experience.
Desirable Criteria
  • Previous community experience.
  • Flexible approach to work.
Skills
  • Effective interpersonal and communication skills.
  • Ability to maintain high standards of care.
  • Clinical competency, including catheter, PICC line, wound, leg ulcer care and palliative care.
  • CRT working.
  • Ability to work unsociable hours and travel between patients’ homes.
Knowledge
  • Evidence of ongoing professional training.
  • Knowledge of professional codes of conduct.
  • Broad knowledge of community nursing skills and competencies.
  • Welsh speaker desirable.
