Community Staff Nurse – Out of Hours

The post holder will participate in the delivery of nursing care and support to clients within the community setting and have the necessary confidence and competence to work autonomously. To work under the direction of the Community Nurse Team leader and to be responsible for assessment and management of the health needs of an identified population including end of life care and patients with long term conditions and/or complex nursing needs.
To support and wherever possible maintain patients in their own preferred place of care, promoting self‑care enabling them to maximise their independence and optimise their quality of life 7 days per week throughout the year.
Main duties of the jobAn exciting opportunity is available for a dedicated nurse to deliver high‑quality clinical care to patients in their own homes. Working under the direction of the Community Nurse Team Leader, you will be responsible for assessing and managing the health needs of a defined patient population, including those with long‑term conditions, complex nursing needs and end‑of‑life care, supporting patients to remain in their preferred place of care wherever possible.
You will make complex clinical decisions to manage patient care safely and effectively, particularly within an out‑of‑hours environment. This includes understanding escalation processes, triaging all communications using electronic patient records, and coordinating workload across teams to ensure care is prioritised according to clinical need. Effective delegation and demand management are essential, alongside the ability to support long day staff and allocate work across sectors.
The role also involves providing reassurance, advice and practical support to patients, families and carers, ensuring high standards of compassionate care. You will contribute to maintaining staff safety by supporting lone working systems and processes. This is a varied and rewarding role requiring strong clinical judgement, organisational skills and the ability to work autonomously while remaining part of a wider, supportive team.

This vacancy is open to those who have a current NMC PIN to practice as a Registered Nurse in the UK.
Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.

Person Specification Shortlisting- Registered Nurse with current NMC registration (Part 1 Adult / General Level 1 or
2) - Be an effective team member who can relate to colleagues at all levels
- Ability to coordinate your workload and supervise members of the team
- IT skills - experience in electronic patient records system
- Ability to be flexible across the needs of the out of hours service
- Prescribing qualification or the willingness to work towards
- Demonstrate initiative to actively problem solve
- Communicates succinctly, clearly and accurately both verbally and in writing
- Demonstrates evidence based knowledge
- Ability to travel to different geographical locations across the Trust footprint in line with service needs
- Ability to use reflective practice to improve the standard of care

- Full UK driving license with willingness to use own car for work purposes
