Community Staff Nurse - Milnthorpe

Role Overview

Milnthorpe District Nursing Team (now based at Yealand Conyers) and the Community, Cancer, Diagnostics & End of Life Division (CCDE) are recruiting a full‑time permanent Band 5 community staff nurse. The role supports an evolving district nursing service and requires a dedicated, patient‑focused clinician.

• Provide high‑quality care to patients with a wide range of medical conditions in the community setting.
• Conduct complex nursing assessments and develop and deliver programmes of care for patients and carers.
• Support the 24‑hour district nurse service to ensure continuity of care.
• Collaborate with health and social care colleagues, voluntary sector workers and informal carers to promote excellent care.
• Participate in the team’s commitment to staff development and patient‑focused practice.

• Band5 nursing qualification and registration.
• Full UK driving licence and access to a car.
• Excellent communication skills and the ability to work independently and as part of a multidisciplinary team.
• Experience in community nursing is desirable.
• Commitment to providing patient‑focused care and continuous professional development.

The role will see you working from the following sites:
Furness General Hospital (FGH) in Barrow, Royal Lancaster Infirmary (RLI), Westmorland General Hospital (WGH) in Kendal, Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
