Dorset Health Care – Community Staff Nurse

We are looking for a full‑time Community Staff Nurse for a 12‑month secondment to join the Blandford team. The role is based in the community and involves supporting patients in their own homes.

• Provide a high‑quality, person‑centred approach to care delivery that considers safety, privacy, dignity and the Trust’s values.
• Undertake assessment of patients’ care needs and develop programmes of care as part of a wider multi‑disciplinary team.
• Manage both acute and chronic nursing needs, developing strong therapeutic relationships with patients.
• Work closely with community partners to achieve the best possible health outcomes.
• Identify care needs, monitor patients’ conditions, record observations and support the development of wellbeing care plans.
• Promote independence and help patients avoid unnecessary hospital admission.
• Deliver exceptional end‑of‑life care, including symptom management and supporting patients in achieving their preferences.

• Full‑time, 12‑month secondment.

• Current registration as a Registered Nurse in the UK.
• Employment requires an Enhanced Disclosure and Barring Service (DBS) check (cost covered by the Trust).
• Applicants who hold a DBS update service subscription may present a valid DBS certificate.
• Eligibility: current Skilled Worker sponsorship holders are ineligible to apply.

This is a fixed‑term role and will be offered on a secondment basis only to nurses who are current NHS staff. Permission to be released on secondment must be obtained before applying.
