Advanced Nurse Practitioner

We are looking for an experienced and enthusiastic Advanced Nurse Practitioner to join our friendly and supportive primary care team. This is an exciting opportunity for an ANP who is passionate about providing high-quality, patient-centred care and enjoys working as part of a wider multidisciplinary team.

The successful candidate will provide autonomous assessment, diagnosis and management of patients with a range of acute and long‑term conditions. You will use your advanced clinical skills to support patients, improve access to care and work alongside our GPs, nurses and wider practice team.

The Advanced Nurse Practitioner will work as part of the practice team to provide safe, effective and high-quality care to patients. Main duties will include:

• Assessing, diagnosing and treating patients with a range of acute and long‑term conditions.
• Carrying out clinical assessments and making appropriate treatment decisions.
• Prescribing and managing medicines within their scope of practice.
• Supporting patients with ongoing health needs and care plans.
• Providing health advice, preventative care and health promotion.
• Working closely with GPs, nurses and the wider healthcare team to provide joined‑up patient care.
• Making referrals where appropriate and following clinical pathways.
• Keeping accurate patient records and ensuring clinical documentation is completed.
• Taking part in audits, service improvements and practice meetings.
• Supporting the learning and development of colleagues where appropriate.
• Keeping up to date with clinical guidance, training and professional requirements.

• Work autonomously to assess, diagnose and treat patients within your scope of practice.
• Manage a range of acute presentations and long‑term conditions.
• Undertake clinical assessments, examinations and reviews.
• Develop and implement appropriate treatment plans.
• Prescribe medications safely in line with current legislation and professional guidance.

• Recognise and manage patients who are deteriorating or require urgent intervention.
