Community Staff Nurse
Job Overview
This is a fixed term / secondment position until 26.06.2027 due to maternity cover. Candidates must obtain permission from their current line manager before applying. Welsh students must apply via Streamlining where all available jobs are listed.
We are seeking highly motivated, passionate Band 5 Community Nurses who are keen to develop into one of the following roles:
- Band 6 District Nurse (through District Nurse Qualification)
- Chronic Condition Case Manager (through disease‑specific training)
- Nurse Practitioner (through modular training)

Main Duties of the JobExcellent communication skills and ability to provide clinically effective care within the District Nurse Team. The West Division of Betsi Cadwaladr University Health Board delivers district nursing services across Anglesey and Gwynedd, working in an integrated way with partner organisations to provide high‑quality, evidence‑based care that keeps patients in their own homes and communities, averts hospital admissions, empowers self‑management and implements seamless, patient‑centred care.
Must be able to travel between patient homes within towns and rural areas in a timely manner. Ability to speak Welsh is desirable;
Welsh or English speakers are equally welcome.

PersonSpecification Qualifications
- Registered nurse or within 2 months of gaining PIN.
- Knowledge of Primary Care.
- Working towards diploma or degree. Previous community experience.
- Effective interpersonal and communication skills.
- Ability to maintain high standards of care.
- Clinical competency.
- Flexible approach to work.
- Evidence of ongoing professional training.
- Knowledge of professional codes of conduct.
- Broad knowledge of relevant community nursing skills and competencies.
- Meets all desirable criteria as per attached job description/person specification.
- Working unsociable hours.
- Ability to travel between patient’s homes in a timely manner.
- Welsh speaker.
