Diabetes Specialist Nurse – Diabetes

The diabetes team based at Kidderminster Hospital provides a wide variety of diabetes services mainly on an outpatient basis while also supporting inpatient and day‑case areas. This part‑time role (2 days a week, including Friday) requires autonomous care of a mixed caseload of patients under consultant care and those referred directly to the Diabetes Nursing Service. You will work with community nursing teams, GP practices, and wider multidisciplinary teams to deliver comprehensive diabetes care.

Key Responsibilities
  • Manage and review your own caseload, scheduling appointments and coordinating with GP practices and community services.
  • Provide direct patient care in outpatient clinics, including complex diabetes, pregnancy with diabetes, and paediatric transition into adult services.
  • Support patients, with technology such as hybrid closed‑loop systems and continuous glucose monitoring.
  • Deliver and participate in structured diabetes education programmes.
  • Advise patients, families and health professionals on effective diabetes management, avoiding unnecessary hospital admissions.
  • Maintain accurate and timely clinical documentation in line with Trust policy and NMC guidelines.
  • Engage in audit, research, quality improvement and policy development initiatives.
  • Attend training and contribute to staff development and record keeping for ESRS.
Person Specification – Experience
  • Experience supporting people with diabetes in managing their condition.
  • Assessment skills and experience of work as a specialist nurse.
  • Experience with hybrid closed‑loop or continuous glucose monitoring systems.
  • Experience in teaching and managing diabetes and pregnancy.
Qualifications
  • Pass grade in GCSE or O‑level English and Maths.
  • NMC registration.
  • Post‑graduate diabetes course such as Insulin for Life, Masters in Diabetes Care, Topical, ENB 928, or equivalent study.
