Practice Nurse
Job Summary
We are looking for an experienced, enthusiastic, flexible, committed and motivated Practice Nurse to join our team at Y Felinheli and become involved in all aspects of the Practice. The role involves providing nursing diagnoses, discharge or independent referrals, managing chronic disease autonomously, presenting care alternatives to patients, and participating in the development of management and prescribing protocols.
Responsibilities- Provide assessment, screening, and treatment services and health education advice.
- Organise and coordinate the provision of nursing services, including delegation of duties to other nursing staff.
- Deliver nursing treatments independently or in partnership with general practitioners according to agreed protocols.
- Provide general and specific health screenings, blood pressure checks, urinalysis, routine immunisations, contraception, and minor injury care.
- Triage and treat patients seeking care, making necessary referrals to secondary care or other members of the primary health care team.
- Maintain clinical areas, restock supplies, and conduct pathological specimen collection and investigation procedures.
- Ensure accurate documentation of consultations and treatments in the clinical computer system and compliance with reporting requirements.
- Participate in practice meetings, policy development, and the formulation of practice philosophy and protocols.
- Provide student and staff education, including induction of new practice staff and development of an in‑house nurse‑training programme.
- Act as infection control lead, ensuring adherence to infection control procedures and conducting agreed audits.
- Support confidentiality, health and safety, equality and diversity, and professional development requirements.
- Registered General Nurse qualification.
- Experience in primary care nursing duties, including chronic disease management.
- Knowledge of nursing protocols and the ability to work independently within agreed parameters.
- Computer literacy and the ability to work as part of a team.
- Bachelor of Nursing or equivalent (preferred).
- Training in chronic disease management (desirable).
- Ability to converse in Welsh (desirable).
Maintain continued education through courses and study days. Participate in annual appraisals, develop a personal learning plan, and support the professional development of the nursing team.

About the PracticeThe Practice is based in Y Felinheli, Gwynedd, with an additional site in Menai Bridge, Anglesey. With 7,000 patients, the practice is served by 2 salaried GPs, 1 ANP, 2 practice nurses, and 2 healthcare assistants.
