Community Nurse – Sirona Care & Health

Are you an enthusiastic and passionate Nurse looking for a fresh start? Come and join Sirona as a Community Nurse, a role where you really make a difference. We are a forward‑thinking team that values you as an individual by supporting your development and investing in you. Integrated Neighbourhood Teams are made up of nurses, therapists, paramedics and support workers who work together to support service users on their healthcare journey.

This position is part of a busy nursing team that supports service users who have nursing needs in their own homes. Flexible working options (part‑time or full‑time) are available and can be discussed at interview.

Responsibilities
  • Provide holistic and specialist nursing care in the community setting.
  • Work within a multi‑disciplinary team to deliver patient‑centred care.
  • Manage a caseload and tailor interventions to individual needs, including health education and promotion.
  • Apply evidence‑based practice, clinical outcomes measures and audit processes.
  • Act autonomously when appropriate, knowing when to seek advice from senior staff.
  • Ensure effective communication – written, verbal and non‑verbal – and adapt for individuals with communication difficulties.
  • Implement health, safety and risk management policies.
Qualifications & Experience
  • BSc or equivalent in Adult Nursing & NMC registration.
  • Evidence of continuing professional development and commitment to lifelong learning.
  • Clinical competency in community nursing, including experience of the application of health, safety and risk management policies.
  • Experience working with a range of health and social care professionals.
  • Knowledge of evidence‑based practice, local and national standards.
Professional Skills
  • Effective communication skills – written, verbal and non‑verbal.
  • Ability to make clinically reasoned judgments for a variety of conditions.
  • Strong time‑management and caseload management.
  • Problem‑solving and independent working.
