Taunton Deane West Primary Care Network is seeking an experienced and motivated Advanced Nurse Practitioner to join our multidisciplinary team in an Additional Roles Reimbursement Scheme ARRS funded post. The role is hosted by Lister House Surgery on behalf of the Primary Care Network and will support the delivery of high quality patient-centred care.
You will work as an autonomous clinician providing advanced assessment, diagnosis, treatment and management for patients presenting with acute illness, long term conditions and complex health needs. You will work closely with GPs, practice nurses, pharmacists, first contact practitioners and other members of the multidisciplinary team to provide safe, effective and timely care.
The successful candidate will help improve access to primary care, support prevention and wellbeing and contribute to the continued development of integrated services across the Primary Care Network. You will play an important role in improving patient outcomes, reducing health inequalities and delivering proactive coordinated care.
Working pattern
Tuesday 8:30am to 6:00pm
Thursday 8:30am to 2:30pm
Friday 9:00am to 5:00pm and 6:30pm to 8:00pm (Late night can be remote)
Main duties of the jobWe are looking for an experienced and motivated Advanced Nurse Practitioner to join our PCN team. You will work at Lister House Surgery to provide high quality patient-centred care for people with acute and long term conditions.
You will work as an autonomous clinician using advanced clinical assessment, examination, diagnosis and treatment skills. You will support the delivery of safe, effective and accessible primary care while working closely with clinicians and the wider multidisciplinary team.

Provide autonomous assessment, diagnosis, treatment and management of patients presenting with undifferentiated and undiagnosed conditions.
Manage acute presentations and long term conditions within your scope of practice.
Prescribe safely and appropriately where qualified as an independent prescriber.
Request, interpret and act on investigations.
Develop management plans and make referrals where appropriate.
Provide health promotion, prevention and patient education.
Work collaboratively with the practice and the wider multidisciplinary team.
Support service development, quality improvement and clinical audit.
Maintain accurate clinical records using electronic patient record systems.
Act as a resource and support learning for colleagues where appropriate.
Participate in safeguarding, infection prevention, clinical governance and continuing professional development.

Job responsibilitiesJob role:
The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills at an advanced level, for patients within the general Primary Care Network. The post holder will demonstrate critical thinking in the clinical decision-making process.
They will work collaboratively with the general Primary Care Network team to meet the needs of patients, supporting the delivery of policy and procedures. In order to work at this level, NMC requirements for advanced Primary Care Network must be met
