As part of our multidisciplinary team, you'll deliver high-quality, patient-centred care across a broad range of practice nursing services, including long-term condition management, health promotion, immunisations, cervical screening, wound care and preventative healthcare. Job responsibilities Some of the key responsibilities of the job:
  • Assess, plan and manage care for patients with long-term conditions, promoting self-management and healthy lifestyles.
  • Deliver vaccination and immunisation programmes, including travel vaccinations where appropriate.
  • Undertake cervical screening, wound care and other routine practice nursing procedures.
  • Provide health promotion, screening and preventative care to improve patient outcomes.
  • Maintain accurate clinical records and ensure care is delivered in line with professional standards and best practice.

Person Specification Qualifications Essential
  • Current NMC registration as a Registered Nurse.
Experience Essential
  • Experience working as a Practice Nurse within a primary care setting.
  • Experience managing long-term conditions and delivering health promotion and preventative care.
Knowledge,

Skills and Abilities

Essential
  • Competence in immunisations, wound care and cervical cytology
  • Excellent communication and interpersonal skills
  • Ability to work independently while collaborating effectively within a multidisciplinary team.
  • Commitment to continuous professional development and evidence-based practice.
