Vascular ACP or Specialist Nurse

An exciting opportunity has arisen within the Surgery and Cancer Division at Mid Cheshire Hospitals NHS Foundation Trust to recruit an experienced Advanced Clinical Practitioner (ACP)/Senior Vascular Specialist Nurse to lead and further develop the Trust's Vascular Specialist Nursing Service. The post holder will provide expert autonomous assessment, diagnosis and management of patients with complex vascular conditions across inpatient, outpatient and community pathways.

Working closely with consultant vascular surgeons and the multidisciplinary team, they will deliver high-quality, evidence-based care, improve patient outcomes and ensure seamless care across the Cheshire & Merseyside Vascular Network. The successful candidate will provide professional and clinical leadership, contribute to service transformation, governance, audit, education and quality improvement, while supporting the development of innovative nurse‑led services. They will play a key role in strengthening service resilience, workforce development and improving outcomes for patients with increasingly complex vascular needs. The post holder will work as an Advanced Clinical Practitioner (ACP)/Senior Vascular Specialist Nurse, providing expert clinical leadership and advanced autonomous practice across the vascular pathway.

They will demonstrate a high level of clinical autonomy, accountability and complex decision‑making in the assessment, diagnosis, treatment and management of patients with vascular disease, delivering evidence‑based, patient‑centred care. Acting as the Trust's clinical expert for vascular nursing, the post holder will provide specialist advice and support to patients, carers and the multidisciplinary team, working collaboratively with consultant vascular surgeons, community services and the Cheshire & Merseyside Vascular Network to ensure coordinated, high-quality care.

The post holder will independently assess, investigate and manage patients, request and interpret diagnostic tests, initiate treatment plans and provide expert care across inpatient, outpatient and ambulatory settings. They will contribute to service improvement through leadership, governance, audit, education, workforce development and research in line with the four pillars of Advanced Clinical Practice., The post holder will work as an autonomous Advanced Clinical Practitioner/Senior Vascular Specialist Nurse, providing expert assessment, diagnosis, investigation, treatment and ongoing management of patients with complex vascular conditions across inpatient, outpatient and ambulatory care settings.

The post holder will independently manage a defined caseload, demonstrating advanced clinical reasoning, professional accountability and expert decision making within their scope of practice. The post holder will:

• Undertake comprehensive advanced clinical assessment, diagnosis, treatment planning and evaluation of patients with arterial and venous disease, diabetic foot complications, vascular ulcers, aneurysm surveillance and post-operative vascular conditions.
• Independently manage nurse‑led vascular clinics, follow‑up clinics, surveillance programmes and urgent patient reviews, ensuring timely intervention and continuity of care.
• Request, interpret and act upon appropriate diagnostic investigations including vascular imaging, pathology and physiological assessments within agreed competency frameworks.
• Independently prescribe medicines (where qualified) and initiate, review and adjust treatment plans in accordance with evidence‑based practice, national guidance and local policies.
