West Leeds Primary Care Network is recruiting a prescribing Nurse, Paramedic or Nurse Practitioner to join their Frailty Service. The role suits an Advanced Practitioner with broad nursing experience, with a strong Independent Prescriber background, especially for acute home visiting.
You will work in a MDT with peer support, assess frailty in patients over 65 at home or in care settings, develop care plans, and support GPs by delivering timely home visits to relieve practice pressures.
