We are seeking an experienced and compassionate Medical Lead
to provide clinical leadership and expert medical care across our inpatient community units in Swindon and North Wiltshire. This is an exciting opportunity to shape the future of integrated care services for older, frail patients with complex health needs, while supporting the development of a high-performing multidisciplinary team.

Reporting to the Medical Director, BSW
, you will play a key leadership role within HCRG Care Group, driving excellence in patient care, supporting medical colleagues through job planning and appraisals, and contributing to the strategic development of our inpatient services.

About the Role

As Medical Lead, you will provide medical oversight and care for patients within our inpatient community units, focusing on older adults living with frailty, multimorbidity, and advanced clinical needs. Working closely with multidisciplinary teams, you will help deliver safe, effective, person-centred care that supports recovery, rehabilitation, and timely discharge.

You will be based predominantly across:

  • Swindon Intermediate Care Centre - 60 rehabilitation and stroke beds across two wards.

Most patients are transferred from Great Western Hospital as a step-down from acute care, with others admitted directly from community services. This vital role helps bridge the gap between hospital and community-based care, ensuring the best possible outcomes for patients.

Key Responsibilities

Working within a multidisciplinary team of Advanced Practitioners, Consultant Practitioners and therapists, you will provide expert clinical leadership and care for patients with urgent, complex and frailty-related healthcare needs. You will play a key role in assessing, diagnosing and managing patients referred through the Urgent Community Response service and wider clinical pathways, supporting individuals who would otherwise require hospital admission or who need timely discharge from acute care.
