multidisciplinary teamwork. The Role As a Diabetes Specialist Nurse, you will play an integral role in improving the quality of life for patients living with diabetes. Working within a highly experienced community diabetes team, you will provide patient education, lead service development, and collaborate with primary and secondary care colleagues to ensure the best possible outcomes. Your clinical expertise and leadership will contribute to managing a complex and autonomous caseload.
Provide tailored, structured education to empower patients and support self-management. Contribute to multidisciplinary team (MDT) meetings to develop holistic care plans. Exhibit advanced clinical skills in managing a diverse, complex patient population.
Skills and Qualifications
- Registered Nurse qualification with active NMC registration and five years of nursing experience.
- Post-graduate diabetes training at level 6 or 7, with structured education accreditation desirable.
- Independent prescribing (V300) and a teaching or mentoring qualification, Experience in community diabetes care and autonomous caseload management.
