Location: Union City, CA

We are currently seeking a compassionate, organized, and team-oriented individual to join our Home Health team as a Full-Time or Part-time Physical Therapist in Union City, CA.

As a Physical Therapist (PT), you will collaborate with a dedicated team of healthcare professionals to evaluate patients’ physical health, develop individualized treatment plans, and improve their mobility, strength, and overall quality of life.

• Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms indicating a change in patient condition to Physician, Case Manager, and/or Director of Nursing.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.

• Current Physical Therapist (PT) License to practice in California (Required).
• Experience in Home Health or Hospice care – 1 Year Required.
• Current CA Driver’s License.
• Ability to travel within a 15-25 mile radius on a regular basis.
