Typical
Duties Include:
- Collaborating with the patient’s care team to manage patient care, including making referrals, updating medication changes, and sharing lab and procedure results.
- Ensuring accuracy in documentation, gathering information from patients, and sharing relevant data with providers.
- Participating in efforts to slow kidney disease progression, with opportunities to learn and grow within our supportive team.
