Intensive In-Community Clinician (IIC) – Owen Health Care Essential Duties and Responsibilities
The Intensive In-Community (IIC) Clinician provides community-based mental health services to children/adolescents and their families, delivering strengths-based, family-centered interventions in home and community settings. Services are provided in accordance with NJ Children’s System of Care (CSOC)/Perform Care expectations (as applicable), agency policies, and all confidentiality and documentation standards.
Clinical Services (In-Home / Community-Based)- Provide individual and family therapy in the home and community (and via telehealth when permitted).
- Complete clinical intakes, biopsychosocial assessments, risk screenings, and ongoing reassessments as required.
- Develop, implement, and update individualized treatment plans with measurable objectives in collaboration with the youth, caregiver(s), and treatment team.
- Use evidence-informed, developmentally appropriate interventions (e.g., CBT-informed skills, trauma-informed care, behavioral strategies, parenting support, crisis de-escalation).
- Provide psychoeducation to youth/caregivers on mental health symptoms, coping skills, emotional regulation, family communication, and behavior support strategies.
- Support school functioning and community stability through coordination and problem-solving with involved systems (as authorized).
- Conduct risk assessment (including suicide/self-harm and safety risks) and implement safety planning with caregivers and youth as indicated.
- Provide crisis intervention, de-escalation, and coordination with emergency services/mobile crisis when clinically necessary.
- Maintain timely communication with supervisors/leadership regarding urgent/high-risk situations per agency protocol.
- Coordinate care with collateral providers and systems (e.g., schools, primary care, psychiatry, DCPP, juvenile justice, case managers, mentors), with appropriate releases.
- Participate in treatment team meetings, Child Family Team (CFT) meetings, case conferences, and other interdisciplinary meetings as required.
- Support linkage to community resources and natural supports (after-school programs, family supports, transportation resources, housing/benefits supports as applicable).
- Complete documentation accurately and on time in the agency EHR, including:
- Intake/assessment documents
- Treatment plans and updates
- Progress notes for every contact (including collateral contacts)
- Discharge summaries and transition plans
- Maintain records consistent with payer requirements (e.g., CSOC/Perform Care), agency policy, and applicable NJ regulations.
- Protect confidentiality and follow mandated reporting requirements and ethical standards.
- Maintain a consistent schedule aligned with family availability (including some evenings as needed).
- Attend clinical supervision, trainings, staff meetings, and required in-services.
- Participate in on-call/coverage expectations if assigned.
- Perform other duties as assigned to support program operations and quality care.
- Master’s degree in Social Work, Counseling, Psychology, Marriage & Family Therapy, or related behavioral health field.
- A valid and active LAC, LSW, LCSW, or LPC license in the state of New Jersey is required.
- Experience providing services to children/adolescents and families preferred.
- Knowledge of community-based systems (CSOC/Perform Care, DCPP, schools, juvenile justice) preferred.
- Strong clinical judgment, crisis management skills, and ability to work independently in the community.
- Strong written and verbal communication; ability to meet documentation deadlines.
- Valid driver’s license and reliable transportation (if travel is required).
- Ability to pass required background checks and comply with agency onboarding requirements.
