Stern Therapy Consultants is hiring a PRN Occupational Therapist at Autumn Lake at Union
. In this skilled nursing rehabilitation role, you'll identify what stands between residents and their everyday activities, then provide treatment and practical strategies that help them participate more independently.

• Assess self-care performance, functional transfers, upper-extremity abilities, cognition, and activity tolerance.
• Develop individualized plans of care based on evaluation findings, resident priorities, and anticipated discharge needs.
• Use purposeful activities to improve daily living skills and address limitations affecting task completion.
• Recommend adaptive equipment and educate residents and caregivers on safe techniques and compensatory strategies.
• Monitor functional gains, revise treatment goals, and supervise COTA services.
• Maintain thorough clinical records and communicate recommendations to nursing and the interdisciplinary care team.

• $50–$55 per hour.
• PRN work scheduled according to your availability and facility coverage needs.
• Unlimited referral bonus, with up to $1,000 per successful referral.

• Active New Jersey Occupational Therapist license.
• Strong evaluation, clinical reasoning, and treatment planning skills.
• Effective communication and accurate, timely documentation.
• Skilled nursing or post-acute experience is a plus; new graduates are welcome to apply.
