Location:
Union, NJ. Department: RIST 5 – Hudson and Essex County and Supported Housing Union (Union County).
Work hours:
Monday-Friday (9am-5:00pm) – Full time (40 hours weekly). Salary: $52,900 - $69,500.
As part of the CSS team, provides nursing services, direct-support services, care management, education and primary health care to persons served. Assures that medical needs are being addressed in the community. Provides support and education to other team members in nursing/medical treatment and health care. Nursing practice supervision is provided by the RN, or in group format quarterly by the Bridgeway Bridge-to-Wellness Medical Advance Practice Nurse.
Responsibilities- Provide nursing services, direct-support services, care management, education and primary health care to persons served.
- Ensure medical needs are addressed in the community.
- Offer support and education to team members in nursing/medical treatment and health care.
- Collaborate with supervisory RN or NP for nursing practice supervision.
- High school diploma or equivalent; must have graduated from an accredited nursing training program.
- Possess and maintain licensure, in good standing, in the State of NJ as an LPN.
- One year of technical nursing training supplemented with two years of paid work experience in the community.
- Valid driver’s license with no more than one moving violation within the past 12 months; vehicle required.
