Job Title
Nurse Practitioner – Palliative
LocationUnion, Blacksburg, Gaffney area, South Carolina.
About the RoleYou are a key provider delivering high‑quality care to patients across homes, facilities, and through telehealth. This full‑time salary‑based position includes a monthly bonus opportunity.
Shift Schedule- 8‑hour shift Monday‑Friday, 8:00 am–5:00 pm
- 10‑hour shift Monday‑Thursday, 7:00 am–6:00 pm
- Evaluate high‑risk patients with complex medical needs and develop individualized care plans.
- Coordinate care activities with interdisciplinary team members to ensure continuity and optimal outcomes.
- Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, and health coaches to coordinate patient care.
- Participate in team meetings to discuss patient progress, treatment plans, and care coordination.
- Foster effective communication and collaboration to facilitate seamless patient management.
- Provide care in homes, facilities, telehealth, clinics, and specialty settings while meeting staff efficiency expectations.
- Document care management activities accurately to support patient plans and regulatory compliance.
- Educate patients and families about conditions, treatment plans, self‑management strategies, and preventive care.
- Offer guidance to enhance understanding of healthcare options and promote active participation.
- Adhere to all relevant regulations, including HIPAA, Medicare guidelines, and internal policies.
- Monitor quality measures, identify gaps, and implement corrective actions as needed.
- Valid Nurse Practitioner, Physician Assistant, or Physician license in the hiring state, in good standing.
- Experience providing patient care in home health, clinic, facility, telehealth, or specialty settings.
- Strong communication, collaboration, and interpersonal skills.
- Ability to evaluate high‑risk patients, develop care plans, and coordinate complex care activities.
- Commitment to compassionate, patient‑centered care and a culture of excellence.
- Excellent organizational and time‑management skills with the ability to multitask and prioritize.
- Proficient in written and verbal communication and basic computer use.
- Demonstrated leadership ability and a cooperative work environment.
- Licensed driver with a reliable, insured vehicle; flexible travel between offices and facilities.
- Ability to work flexible hours as needed.
