Health Home Care Coordination Supervisor Level 1
Provides care coordination staff supervision to ensure families receive quality family‑centered case management services. Fosters patient advocacy for children with special health care needs through skilled, professional case management. Works toward achieving the six core care‑management functions of the health home, ensuring children meet goals of comprehensive patient/family focused plans. Oversees education and training of case coordinators, identifies resources and referrals, and monitors quality improvement measures to initiate changes in processes and policy.
Supports St. Mary’s mission by complying with NYS DOH and Health Homes regulatory guidelines and agency standards.
- Oversees and monitors performance of the care coordination staff to ensure that Program participants receive quality and timely services.
- Identifies needs and ensures access for training and ongoing staff education on processes, resources, advocacy and family centered case management services.
- Guides staff on conflict resolution, advocacy issues and promotes relationship building with families and provider agencies.
- Initiates process and policy change to ensure the delivery of family centered case management services.
- Utilizes the Child and Adolescent Needs Assessment (CANS) to identify child’s medical, social, educational and psychological needs; works with Care Coordinator team members to develop comprehensive plans of care that meet the child’s identified needs.
- Monitors and assists case managers with developing plans of care according to patient’s needs and program guidelines; ensures case management and related services are delivered in accordance with patient’s care plan.
- Participates in and initiates case conferences in a timely manner to advocate for the patient and facilitates the approval of case management and related services.
- Provides oversight of the completion of all required and necessary visits/contacts to ensure that the child is receiving coordinated services to meet the goals of the plan of care; tracks staff documentation and ensures compliance with program requirements and regulations.
- Maintains collaborative relationships with all internal personnel and outside agencies to ensure ongoing quality care coordination.
- Ensures monthly productivity and billing statistics are in accordance with Case Management program and department standards.
- Assists with case load coverage as needed.
- Participates in supervisor meetings biweekly and meets with Director monthly as needed; reports on case status and staff performance.
- Participates in quality improvement activities and ensures incidents and complaints are reported and meet program regulatory requirements.
- Monitors and reports all incidents and complaints relating to patient-family health and safety; follows the Mandated Reporter requirements and participates in the Incident Reporting Committee (IRC) monthly.
Bachelor’s degree from an accredited college or university with a minimum of two years experience in Health and Human Services; or R.N. licensure with one year nursing experience; pediatric and/or case management experience preferred; or M.S.W. from an accredited school of social work with a minimum of one year experience in Health and Human Services, preferred. Valid driver’s license required.
