Position: Home Health Aide (HHA)
Shift: PRN (as needed)
Pay Rate: $17/hour
Job Summary: Townhouse Center for Rehabilitation & Nursing is seeking a compassionate and reliable Home Health Aide (HHA) to join our team on a PRN basis. The ideal candidate will assist residents with transportation to medical appointments, provide support during outings, and ensure safety and comfort at all times.
Key Responsibilities- Escort residents to and from clinic appointments and dialysis sessions.
- Monitor and prevent falls or altercations, intervening promptly when needed.
- Assist residents in preparing for appointments and ensure snacks are provided during out-on-pass periods.
- Report all incidents and accidents promptly to the Clinical Coordinator or ADNS.
- Represent the facility professionally, adhering to all policies and procedures both on and off the property.
- Current New York State Home Health Aide (HHA) certification.
- Good verbal communication skills and a flexible, dependable attitude.
