East Meadow location

AHRC Nassau provides OPWDD-approved Day Habilitation Services in over 30 locations across Nassau County. We provide no cost door-to door transportation, medical oversight by registered nurses and an engaged staff who provide meaningful and person-centered experiences each and every day.

Qualifications:

  • Must be a NYS Licensed Practical Nurse
  • CPR, fire safety training, and GT feeding training required
  • Six month of hospital or long-term care experience, preferably working with /DD population preferred

Physical Demands/Work Environment

Physical aspects may include but are not limited to lifting and transferring of an adult volunteer.

Primary duties and responsibilities include but not limited to:

Performs tasks and responsibilities within the framework of health counseling and provision of supportive and restorative care under the direction of the RN or MD; assigned to attend to the medical needs of volunteers in the Day Habilitation program; includes, but is not limited to direct administration of gastrostomy tube feedings (upon successful completion of OMRDD course), medication administration, first aid and emergency response;

provides supervision and medical oversight while accompanying volunteers during transit to/from their homes.

Provides supervision and medical oversight for volunteers while on community opportunities; transports volunteers to volunteer job opportunity sites or other community based activities; assists volunteer in achieving success and satisfaction in their community based endeavors; completes all requested medical and program documentation (i.e. Daily Records; Program notes, etc. ) related to the care of the volunteers.
