Continuum Rehab Group provides in‑house therapy with Skilled Nursing facilities in Texas, New Mexico, Arizona, Indiana, and Missouri.

Base pay: $52.00 hr – $58.00 hr. Total compensation includes 401(k), PTO, medical, dental, and vision benefits.

• Graduated from an accredited university.
• Valid license in the appropriate state (or in process).
• Open to therapists at all experience levels.

• Utilizes evidence‑based practice to support clinical interventions.
• Provides rehabilitative, skilled, and medically necessary treatment to patients with physical functioning disorders per regulatory and clinical practice requirements.
• Delivers skilled therapy services to patients requiring rehabilitation.
• Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
• Supervises Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
