** Responsible for managing patient care and treatment (in person, telephonically and/or video).

*** Primary duties may include, but are not limited to:
*** Provides a range of primary, acute and/or specialty healthcare services for patients in clinics, virtually or in home settings.
* In states that do not require collaborative agreements, functions as the primary care provider.
* Orders, interprets and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.
* Provides health instruction and counseling, educating patients on disease prevention and healthy lifestyle choices.
* May work in collaboration with other Physicians, Nurses or other healthcare professionals to treat the needs of the patient(s) as part of an interdisciplinary care team.
* Formulates and documents care plan.
* Prescribes medication or other forms of treatment.
* Participates in On-Call program, which may include after-hours, weekend and holiday calls and visitations.
** Requirements:
*** Requires an MS in Nursing
* Minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.
* Requires a current, active, valid and unrestricted RN license and NP license
* Satisfactory completion of a Tuberculosis test
** The posted compensation range represents the national market average. Compensation for roles located in premium or high-cost geographic markets may fall above this range. This position is bonus eligible based on individual and company performance.
