Come join our Primary Care team! This is a fantastic opportunity to make a difference in the lives of patients seeking day‑to‑day care. Working in a medical office that provides excellent customer service while focusing on patient‑focused care can be incredibly rewarding.
As a part of this team, you will work alongside seven Medical Providers and a team of nurses and medical assistants, collaborating to provide top‑notch care to patients. With an average of 60–100 patients seen each day, this role offers a dynamic environment where you can collaborate with team members to provide comprehensive patient care.
You will handle a variety of health maintenance needs (e.g., diabetes, hypertension, and more), allowing you to develop skills and knowledge that benefit both your professional growth and the well‑being of the patients you will be assisting.
Responsibilities And Duties- 25% Performs initial, ongoing, and functional health status assessment as applicable to the population and/or individual. Collects and documents subjective and objective data from patients, significant others, and communities. Reviews data obtained by other members of the health care team and incorporates it into assessment. Identifies and communicates assessment findings to appropriate people via referrals, rounds, huddles, reports, and care conferences.
- 25% Based on nursing diagnoses and collaborative problems, documents planned nursing interventions to achieve outcomes. Initiates and/or coordinates care plans and individualizes critical paths/medical management plans. Promotes cost‑effective care by minimizing fragmentation and maximizing coordination.
- 20% Evaluates and documents response to nursing interventions and achievement of outcomes at appropriately determined intervals; revises plan of care based on evaluative data.
- 20% Delegates tasks in accordance with specific criteria and state statutes. Reviews work performed by those to whom it is delegated. Coordinates continuity of care as indicated by patient, family, and community needs.
- 10% Actively participates in process improvement activities to achieve targeted measures of clinical quality, customer satisfaction, and financial performance. Acts as patient advocate and participates in committees and self‑development programs.
- High School Diploma or GED (Required)
- BLS – Basic Life Support – American Heart Association
- LPN – Licensed Practical Nurse – National Council of State Boards of Nursing
- Must hold a current license from the State of Ohio
- Successfully completed an Ohio Board of Nursing approved medication course
