Interim Health Care Inc. in the South Hills area seeks a Registered Nurse Case Manager to support a full range of home health services. You will conduct in-person patient interviews, assess needs, and coordinate care across nurses, aides, RNs and other providers.
You will educate patients and families on home health care procedures, promote preventive care to reduce readmissions, and ensure continuity of care through effective communication with the care team.
