Carle Health is seeking a BE/BC Family Medicine or Internal Medicine physician to join our Transitional Care Clinic (TCC) at Carle Foundation Hospital in Urbana, Illinois.

• Part-time position with clinic hours M-F, 8:00 a.m.–5:00 p.m.
• Work within the Transitional Care Clinic (TCC) located at Carle Foundation Hospital to assist patients staying on the path to recovery after hospital discharge until they can see their Primary Care Provider.
• Join a team of one physician and one Advanced Practice Provider.
• Develop patient treatment plans directed toward returning patients to a state of wellness, establishing the need for intervention, and evaluating responses to and outcomes of therapy.
• Communicate with other health professionals, families, and referral agencies regarding health status and needs of particular patients or patient groups.
• Opportunity to teach residents through Carle Foundation Hospital's Internal Medicine residency program.
• Opportunity for academic and/or research affiliation with Carle Illinois College of Medicine and related programs.

The compensation range for this position is $138,500 – $140,000. The actual compensation offered will depend on factors including experience, qualifications, location, licenses, shifts worked, and compensation model.
