Overview
Carle Health is seeking a BE/BC Family Medicine or Internal Medicine physician to join our Transitional Care Clinic (TCC) at Carle Foundation Hospital in Urbana, Illinois.
Opportunity Details- Part‑time position
- Clinic hours are M‑F / 8:00 a‑5:00 p
- Conveniently located within Carle Foundation Hospital, the Transitional Care Clinic (TCC) assists patients with staying on the path to recovery after leaving the hospital until they are able to see their Primary Care Provider
- Join a team of one physician and one Advanced Practice Provider
- Develop patient treatment plans directed toward returning patients to a state of wellness, establishing the need for intervention, and evaluating responses to and outcomes of therapy
- Communicate with other health professionals, families, and referral agencies regarding health status and needs of particular patients or patient groups
- Opportunity to teach residents through our accredited Internal Medicine residency program at Carle Foundation Hospital
- Opportunity for academic and/or research affiliation with the Carle Illinois College of Medicine, the nation's first medical school focused at the intersection of healthcare and engineering

The compensation range for this position is $138,500 – $140,000. This represents a good‑faith minimum and maximum range for the role at the time of posting. The actual compensation offered to a candidate will depend on a variety of factors including experience, qualifications, location, training, and compensation model.
