Overview
The Discharge Unit RN provides care for the adult patient population ready for discharge. This may include but is not limited to acute medical, surgical, neuro, and cardiac diagnosis who are medically stable and cleared for discharge by the primary care team.
Qualifications- Certifications:
Licensed RN (IDFPR);
Basic Life Support (AHA) within 30 days - Education:
College Diploma in Nursing - Work Experience:
Nursing experience in acute medical surgical neuro cardiac care (medically stable ready for discharge)
- Promotes evidence-based practice through nursing process, protocols, and procedures
- Assesses patient status through interview, observation, and physical assessment of actual problems and outcomes
- Assesses and chooses teaching strategies appropriate to patient/family age specific needs; adapts teaching based on response, readiness to learn, level of comprehension
- Takes cultural, age-specific, gender, developmental factors into account when teaching patients about procedures, illness, hospital experience
- Identifies and utilizes team resources to solve identified problems or needs; consults appropriate resources within the hospital to provide specialized education
- Completes required documentation as appropriate, including patient chart and event reports; analyzes clinical situations in a systematic way and acts to obtain resources or correct problems to meet or exceed expected outcomes or patient needs
- Advocates with physicians on patient’s behalf when necessary; promptly and efficiently follows up on patient/family requests/needs – anticipates patient/family needs; recognizes need for assistance and utilizes resources, including physician and nurse to nurse consultation in a timely manner
- Demonstrates technical knowledge of equipment and troubleshoots when problems arise; actively promotes and achieves customer satisfaction; provides discharge education; arranging and/or coordinating with other departments to establish discharge transportation; administer medication ordered by the care team

Compensationand Benefits
The compensation range for this position is $32.21 per hour – $55.40 per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered to a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
