LPN/RN Field Associate - Part-Time (Urbana, OH)

Provide quality nursing services to consumers based on each consumer's medical needs and Individual Plan. Ensure operations comply with DODD Medicaid (Title XIX), state licensure, and other regulatory standards.

• Follow established policies and procedures, maintain accurate records, ensure compliance with all trainings and employment requirements.
• Monitor and ensure consumer safety, health, and welfare, including routine assessments, medication administration, treatment of illnesses and injuries, and maintenance of communication with staff of day services.
• Coordinate with program staff for scheduling health professional appointments, liaise with physicians for orders, and maintain communication with pharmacy.
• Participate in admission, discharge, and plan‑of‑care discussions, make referrals to community resources, document all services, and assist consultants during rounds.
• Monitor MAR/TAR reviews for corrective action and develop written plans in coordination with the RN for each consumer, covering corrective diets, preventive health care, personal hygiene, medical measures, and extensive treatment.
• Maintain a clean and safe environment, participate in infection control and ongoing monitoring for delegated nursing, uphold conduct and courtesy toward consumers and staff, attend staff meetings, and maintain medication storage and ordering.
• Assume on‑call responsibilities and perform other duties as assigned.

• Current license as a Practical Nurse or Registered Nurse in Ohio.
• Valid driver's license and vehicle insurance.
• Acceptable police record and background screening.
• At least 1 year of experience caring for adults (preferred).
• Professional liability insurance (recommended).

Compensation: $27.50 per hour.
