Visit

Nurse-RN
  • Perform recertification's, resumptions of care, discharge assessments, and routine client visits.
  • Counsel clients and families about nursing and related needs.
  • Communicate with the Nurse Case Manager about healthcare provider orders, necessary equipment, incident reports, and changes in client condition.

Qualifications Experience

Visit Nurse: 3+ years of nursing experience, preferably in Home Health or similar setting.

Licenses & Certifications
