About the Role:
Registered Nurse Case Manager
As a Registered Nurse Case Manager, you’ll play a key role in supporting the health, happiness, and daily experience of the residents at Wesley at Home.
As a Registered Nurse Case Manager, you will:
- You will have the opportunity to directly and positively impact the health and wellbeing of the people you serve through the provision of professional nursing services.
- Support a case load of hospice patient in an assigned area.
- Provide compassionate, person-directed care to patients and families.
- Full-Time Monday-Friday, Back-up on-call Weekday and weekends
Qualifications:
- Current vehicle insurance.
- Current license as a registered nurse in the state of Iowa.
- Two years of full-time nursing experience required, with one year of hospice experience preferred.
