Home Care RN – UnityPoint Health

Join us as a Home Care RN with UnityPoint Health  a registered nurse, you’ll provide one‑on‑one care to patients in their homes, managing admission, care planning, and discharge with full autonomy.

Sign‑On Bonus: $10,000 – available for new hires.

Shift: Monday – Friday, 8:00 AM – 5:00 PM (with weekend/on‑call/holiday rotation).

Overview

As a Home Care RN, you’ll deliver high‑quality patient care in the patient’s place of residence, using clinical nursing knowledge, physical assessment, teaching, and procedural skills. You will develop and oversee the home health plan under physician direction, ensuring interdisciplinary involvement and optimal patient outcomes.

Responsibilities
  • Make visits to patients’ homes and conduct ongoing interdisciplinary assessment.
  • Perform initial and comprehensive assessments on all patients per agency policies.
  • Conduct nursing assessments during each visit per agency policies.
  • Ensure quality and safe delivery of services per care plan.
  • Use equipment and supplies safely and efficiently.
Qualifications

Education: Graduate of a state‑board approved program for Registered Nurses.

Licensure/Certification:

  • Valid RN license in the state of practice.
  • Valid licensed driver with automobile insurance as required by state or organization.
  • Mandatory reporter for child & dependent adult abuse.
  • Valid Basic Life Support (BLS) Healthcare Provider Card with recertification.

Minimum Experience: One year of nursing experience caring for a similar patient population.

Other: Ability to use standard equipment required for essential job functions.
