• Carry out the prescribed plan of care for the client as detailed in the Service Plan.
• Provide assistance with activities of daily living in personal care cases, including bathing, skin care, toileting, ambulation and transfers; assistive lifts require client-supplied equipment.
• Assist with self-administered medications, with periodic prompts and reminders.
• Provide companionship and sitter activities (walking, reading, visiting, games, conversation).
• Offer independence training or safety supervision upon client request.
• Provide homemaking support: laundry, cleaning, meal preparation, menu planning, shopping, errands, and transportation to appointments.
• Report changes in the client's condition to the supervisor or health professional.
• Take and record vital signs; complete required reports and records on time and accurately.
• Arrive to work as scheduled; adapt to client schedule, preferences, and requests; maintain a clean, well‑groomed appearance.
• Other duties may be assigned.

• High school diploma or GED preferred; ability to add/subtract two-digit numbers and to multiply/divide with tens and hundreds; ability to use units of American money, weight, and volume.
• Language and literacy skills adequate for client communication.
• HHA, PCA or CNA preferred, or comparable and relevant experience and/or training.
• 1+ years of experience caring for a senior in either the acute, long-term care, or community-based care setting preferred.
• Compassionate provider of personal care, homemaking, and companion services.
• Ability to pass a thorough Criminal and Background Record Check.
• A minimum of 4 recent references (2 personal and 2 professional).
• Cleared fitness to work and 2-step PPD test.
• Personal qualities including: compassion for the elderly; honesty, trustworthiness, reliability, and patience.
• Reliable transportation to a client’s home and/or a valid driver’s license and auto insurance if required by the client.

• Demonstrates empathy for vulnerable older adults and values their independence.
• Able to prepare simple meals.
• Able to demonstrate appropriate cleaning techniques for bathrooms and kitchens.
• Language and literacy skills adequate for client communication.

As assigned by Griswold Home Care.

Overtime compensation at time-and-a-half for time worked in excess of forty (40) hours per week.
