Een organisatie in de geestelijke gezondheidszorg in Utrecht zoekt een Persoonlijk Begeleider voor 24 - 36 uur per week. Je bent verantwoordelijk voor de zorg voor cliënten en coördineert hun begeleiding. Je werkt nauw samen met diverse zorgverleners om doelen vast te stellen die de zelfredzaamheid van de cliënten vergroten. De ideale kandidaat heeft een hbo-diploma, werkervaring in de psychiatrie en een praktische, analytische instelling.

Afhankelijk van ervaring en opleiding is salarisschaal FWG 45 of 50.
