Bartiméus Doorn zoekt een Coördinerend Begeleider voor twee woningen bij Doorn. Je bewaakt zorgplannen, coördineert dagelijkse zorg en behartigt bewonersbelangen richting verwanten en betrokkenen.
Je werkt nauw samen met collega-begeleiders, gedragskundige, dagbesteding, medische disciplines, behandelaars en de teamleider. Je hebt een mbo-opleiding niveau 4 in zorg of welzijn en bij voorkeur ervaring in de gehandicaptenzorg.
