Een zorgorganisatie in Utrecht zoekt een ervaren ambulant verpleegkundig casemanager voor hun FACT Jeugdteam. In deze rol werk je intensief met jongeren van 12 tot 23 jaar die vastlopen door psychiatrische problemen en andere uitdagingen. Je bent de spil in het team en biedt behandeling op maat in verschillende omgevingen. Je krijgt veel autonomie en werkt in een organisatie die zich richt op samenwerking en herstel.
Salaris conform CAO GGZ max. € 5.140 bruto per maand bij 36 uur.
