Een zorgorganisatie in Utrecht zoekt een gezinscoach die kan werken met multi-probleem gezinnen en hen ondersteunt bij hun toekomst. Je werkt gemiddeld 32 uur per week en bent verantwoordelijk voor huisbezoeken, het opstellen van begeleidingsplannen en samenwerking met multidisciplinaire teams. Een HBO-diploma in Social Work of vergelijkbaar is noodzakelijk, evenals SKJ-registratie. De rol biedt een salaris van €3.300 tot €5.096 bruto per maand, afhankelijk van ervaring.
