Flits Interim is op zoek naar een opdrachtnemer die een resultaatgerichte bijdrage levert aan de gemeentelijke uitvoering van de Wet maatschappelijke ondersteuning in Utrecht. Deze rol richt zich op bemoeizorg en de aanpak van personen met zwaardere GGZ-problematiek.
Je zult verantwoordelijk zijn voor het oppakken, behandelen en afronden van ten minste 30 nieuwe meldingen in een periode van 10 weken. Dit omvat het opstellen van integraal ondersteuningsplannen en samenwerking met ketenpartners zoals wijkteams en zorgaanbieders.
