Lister zoekt een persoonlijk begeleider/ herstelcoach voor het Gebiedsteam Utrecht Oost Noordoost. Je bent het eerste aanspreekpunt voor cliënten en coördineert zorg met het netwerk. Met een eigen caseload lever je ambulante begeleiding bij mensen met een ernstige psychiatrische aandoening en/of verslaving.
Je werkt zelfstandig vanuit het team, sparren over casussen en draagt bij aan doelen die zelfredzaamheid vergroten. Een jaarcontract met mogelijkheden tot verlenging wordt geboden.
