Interakt Contour in Utrecht zoekt een Persoonlijk Begeleider voor het Gebiedsteam Utrecht Overvecht Noord (24 uur per week). Je bent het eerste aanspreekpunt voor cliënten en coördineert hun zorg met het sociale en zorgnetwerk.
Je werkt aan zelfstandig functioneren, het voorkomen van terugval en het vergroten van vaardigheden. Je werkt samen met huisartsen, zorgprofessionals en familie om doelen te realiseren.
