Lister Schakel-om zoekt een Persoonlijk Begeleider in Utrecht voor zwangerschapsvervanging van 6 tot 8 maanden. De functie vraagt 32 uur per week met een focus op herstel, zelfredzaamheid en het voorkomen van terugval.
Je coördineert zorg rondom de cliënt, werkt nauw samen met het sociale en zorgnetwerk en zet in op ontwikkeling, eigen regie en vertrouwen. Vereist HBO-diploma Social Work/SPH/MWD en relevante ervaring in de psychiatrie of verslavingszorg.
