Lister Nieuwegein Midden zoekt een Persoonlijk Begeleider / Herstelcoach voor 28-36 uur per week. Je krijgt een eigen caseload van ambulante cliënten met Wmo-, BT- of Wlz-indicatie.
Je coördineert zorg, stelt doelen vast in samenspraak met cliënt en sociaal netwerk, en werkt aan zelfredzaamheid en herstel. Wij bieden FWG 45/50, max maandelijks €4.273–€4.716, eindejaarsuitkering en pensioenfonds, plus scholing en woon/werkverkeer- vergoeding; contract voor jaar met uitzicht op verlenging.
