Lister zoekt een persoonlijk begeleider / herstelcoach voor de Nicolaasstraat in Utrecht. Je werkt 32 uur per week en bent eerste aanspreekpunt voor cliënten, met focus op ontwikkeling, zelfredzaamheid en herstel.
Je coördineert de zorg rondom de cliënten uit jouw caseload en werkt nauw samen met zorgnetwerken en ketenpartners. Wij bieden FWG 45/50 salaris, CAO ggz en een jaarcontract met uitzicht op verlenging.
