FPA Utrecht in Den Dolder is op zoek naar een maatschappelijk werker die patiënten in de forensische psychiatrie ondersteunt bij wonen, financiën en nazorg. Je werkt in een hecht multidisciplinair team en levert input voor behandeltrajecten terwijl je de uitstroom tot stand brengt en maatschappelijke ontwikkelingen volgt.
Je bent verantwoordelijk voor het versterken van netwerken rondom de patiënt, samenwerking met verwijzers en ondersteuning bij zorgmaatregelen.
