De Tussenvoorziening zoekt een Individueel Begeleider om bewoners te ondersteunen vanuit Team Leidsche Rijn en Okinawa Beschermd Thuis in Utrecht. Je begeleidt cliënten in hun herstel en zelfstandigheid en bouwt mee aan een groeiende woonlocatie.
Je werkt intensief samen met lokale partners en draagt bij aan een sterke wooncommunity. Een HBO-opleiding Zorg & Welzijn is vereist, plus ervaring met begeleidingsplannen en rapportages.
