Lister zoekt een (senior) persoonlijk begeleider voor Houten Castellum. 28-32 uur per week. Je bent eindverantwoordelijk voor zorgverlening en werkt in een team dat autisme begeleidt en diverse psychische kwetsbaarheden ondersteunt.
Je coördineert zorg rondom de cliënt, werkt nauw samen met het sociale netwerk en zet methodische begeleiding in ter bevordering van zelfstandigheid en herstel.
